Soulstone Health LLC ("we", "us", or "our") operates soulstonehealth.com (the "Site"). We build open-source web applications for the healthcare industry. If you have any questions about this policy, you can reach us at rodney@soulstonehealth.com.

We collect personal information only when you voluntarily provide it. Specifically:

  • Contact form submissions — When you fill out the contact form on our Contact Us page, we collect your name, email address, phone number (optional), and message. This information is used solely to respond to your inquiry.

We do not collect any personal information through passive tracking, and we do not use advertising networks, behavioral analytics, or any form of user profiling.

This Site uses a minimal number of cookies:

PHPSESSID Essential Set by PHP when you submit the contact form. Used only to display a one-time success or error message after the form is submitted. Contains no personal data. Session (deleted when you close your browser)

We do not use any analytics cookies, advertising cookies, or other non-essential tracking cookies. Because the only cookie we set is strictly essential to the functioning of the contact form, no cookie consent banner is required.


5. Third-Party Services

The Site loads Font Awesome icons from Cloudflare's CDN (cdnjs.cloudflare.com). When your browser fetches these assets, Cloudflare may process your IP address as part of standard CDN and security operations. Cloudflare's privacy policy is available at cloudflare.com/privacypolicy.

All other assets (Bootstrap CSS/JS, stylesheets, images) are served directly from our own servers. No other third-party services are loaded on this Site.


6. Data Retention

Contact form submissions are received by email and retained only as long as reasonably necessary to respond to your inquiry. We do not store form submissions in a database.


7. Your Rights
